Getting pills
Can I buy Diane over the counter?
Quick answer
No. Diane-35 (cyproterone acetate + ethinylestradiol) is a prescription medicine, and every pharmacy listing we checked asks for a reseta (Prescription. All birth control pills need one in the Philippines.).[src][src] Its clot risk is higher than levonorgestrel pills, which is one reason doctors want to see you first. Once prescribed, Althea and Cybelle have the same formula for less.
Many Filipinas first took Diane-35 years ago, when pharmacies sold it freely, and only found out it needs a prescription after the 2024 crackdown.[src] Under RA 10918 it was always a prescription medicine.[src] This page explains why Diane-35 in particular is a pill doctors like to check before prescribing.
Why Diane-35 needs a doctor
The European Medicines Agency reviewed Diane-35 in 2013. It found the risk of blood clots in the veins is 1.5 to 2 times higher than with levonorgestrel pills such as Trust (levonorgestrel + ethinylestradiol), and it kept Diane-35 for women with moderate to severe androgen-related acne or excess hair, usually after creams and antibiotic tablets have not worked.[src] EMA also said it should not be taken with another hormonal contraceptive.[src]
That is a different job from "just a pill". If you only want contraception, a doctor may suggest a levonorgestrel pill first. If acne or PCOS is the main reason, Diane-35 may be exactly what they recommend.
What the doctor will ask
Expect a short checklist. It usually takes a few minutes, in person or by teleconsult:
- Do you smoke, and are you 35 or older?[src]
- Do you get migraine with aura (flashing lights or zigzag lines before the headache)?[src]
- Have you or a close family member had a blood clot?[src]
- Your blood pressure. Bring a recent reading if you are doing a teleconsult.
- Are you breastfeeding, or did you give birth in the last 6 weeks?[src]
- What you have already tried for your skin, if acne is the reason.[src]
Answer honestly. These are the questions that keep a rare risk rare.
The prescription and the cheaper twins
| Brand | Formula | From |
|---|---|---|
| Diane-35 | Cyproterone acetate 2 mg + ethinylestradiol 35 mcg | ₱695.75 |
| Althea | Same | ₱537 |
| Cybelle | Same | ₱525 |
Ask the doctor to write the generic name with the brand, for example "cyproterone acetate + ethinylestradiol (Diane-35 or equivalent)", if they are happy for you to use a same-formula brand. That way a pharmacist can fill it when one brand is out of stock. Compare all three in Diane-35 vs Althea.

Frequently asked questions
Kailangan ba ng reseta sa Diane?
Oo. Prescription medicine ang Diane-35 at hinahanapan na ng reseta sa Watsons at Rose. Mas mataas nang kaunti ang clot risk nito kaysa sa ibang pills, kaya mabuti na makausap muna ang doktor.
Is Diane-35 free at health centers?
Health centers usually stock a standard combined pill and a progestin-only pill, not Diane-35. For acne-focused pills, expect to buy them with a prescription.
Can I use my old Diane-35 prescription?
If it still shows the doctor's name, PRC licence and signature, some pharmacies will accept it, but many want a recent one. If it has been years, a quick review with a doctor is worth it anyway.
